Women's Pavilion (Obstetrics) Patients
Congratulations and thank you very much for choosing San Gabriel Valley
Medical Center for this most joyous of occasions! We strive to provide
the best care to all of our patients and are committed to making you and
your baby's stay here a pleasant, comfortable one.

How do I pre-register for delivery?
The process for pre-registration is basically the same as for other procedures
at the hospital. You will fill out a Pre-Admitting Form, which is available
at your physician's office, the hospital's lobby information desk,
or from your OB tour guide, should you attend the tour. You may also
click here to download a PDF file of the form and print one at your convenience.
We ask that you fill out the Information Form as
completely as you can. You may also wish to see a Registration Representative to
pre-sign your admitting forms so they will be ready for you when your
delivery day arrives.
Checking in:
What do I do to check-in on the date of my delivery?
If you arrive prior to 9pm on the day of your delivery, please enter through
the main lobby. If you arrive after 9pm, please enter through the Emergency
Department entrance.
When you arrive, you will be escorted to the Women's Pavilion. A Registration
Representative will complete the admission process at that time.
What do I need to bring with me on the date of my delivery?
Please bring any paperwork you may have from your physician such as orders
and Pre-natal test results as well as your valid ID card (driver's
license, photo ID card, passport, etc.), Social Security number and insurance
card(s). Please bring your completed Pre-Admitting form if you have not
yet and provided it to the hospital.
