Notice of Patient Privacy

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. 

Effective April 14, 2003, the law requires that San Gabriel Valley Medical Center (SGVMC) provide patients with a copy of our Notice of Privacy Practices for Health Information.  We will give you a copy at the time of your first visit as a patient.  If we revise our notice thereafter, we will provide you with the new notice at your next visit.  You may view the notice by clicking on the links below:
